I'm here with Dr. Stephan Mayer, Director of Neuro Critical Care, New York Presbyterian Hospital / Columbia University Medical Center. Dr. Mayer, first of all, can brain injury cases be treated with any hospital or do you require special facilities? >> There's this growing movement towards the specialized neurological intensive care unit, trauma unit. It's like the unit that which differences in and offer the same kind of expertise. Basically, you've got nursing staff and personnel who are experts in neurological assessments. This isn't kind of an everyday critical care problem. So, the expertise can really make a difference. It's a whole body of expertise that focuses on supporting and preserving the brain in a setting of a critical illness. >> Walk us through what happens from the moment you get a call that there has- there has been a traumatic brain injury. >> Number 1 is urgency. We say time equals brain. So, call 911. Get to medical attention, the closest emergency department as fast as you can. The moment the patient gets within ICU, it's the trauma paradigm. Basically, you're gonna have a team of doctors and nurses to send on the patient and right off the back. Question 1, does this patient need to go immediately to the operating room for some form of decompression of blood or bleeding or swelling that's crushing the brain literally. That's an immediate threat for life and that's what we saw with Ms. Gliffords' case. Like 38 minutes to that emergency room and, very rapidly, she was off to the O. R. for what we call a debridement and a hemicraniectomy. Very interesting story. In the mid- '90s, no one was doing this. It makes sense. Basically, lift off and then remove a big piece of the skull, sort of the brain swells. It can swell outward and not crush the inner part of the brain and the brainstem. But more surgeons were reluctant to deal with because the feeling was well, yeah. Maybe this save her life but we won't be doing them any favors. They're probably gonna be left with terrible disability or maybe in a vegetative state which is a prolonged state of unconsciousness. Well, what we didn't know is that the brain is actually much more resilient, has much more capacity to recover than we had previously given a credit for. >> Is there anything else that doctors can do for patients who, perhaps, are not responsive and are not as fortunate as Ms. Gliffords. >> Absolutely. There's been a lot of discussion about a modern experience with the military conflict in Irag, in Afghanistan and what we've come to learn is that brain injury is now the signature injury of modern warfare. Just in the last 5 years, we got descriptions from the military. There are neuro intensive in the military. It's a very important part of what they do to help our soldiers survive. Describing phenomenon that we've never seen before, that it's all come back to help inform what we do state side. One of the biggest messages is that there's a lot more hope and a lot more that we can really do to help restore people to a good quality of life that we had previously imagined. >> Dr. Mayer, thank you very much for discussing the latest treatments in traumatic brain injury. >> You're welcome.
