Breakthrough treatments and innovations in treatment and prevention have dramatically transformed patient outcomes, allowing those with illnesses from cancer to multiple sclerosis to live longer, healthier lives.

Actor and activist Selma Blair, President and CEO of Blood Cancer United Dr. E. Anders Kolb, and Amazon One Medical Chief Medical Officer Andrew Diamond gathered at the TIME100 Health Leadership Forum, moderated by TIME senior health correspondent Alice Park, in New York City on Sept. 9 to discuss how care is being reimagined because of innovation. Amazon One Medical and Blood Cancer United are sponsors of the TIME100 Health Leadership Forum.

It’s an exciting, transformative era in health care, thanks to new insights in genetics, stem cell science, and gene therapies. Blair, who was diagnosed with multiple sclerosis in 2018, recalled her mom telling her that she was fortunate because they have so many more treatments for her condition today. For example, she underwent a bone marrow transplant to halt disease progression that “bought me a lot of time and an attitude shift I really needed,” Blair said. She also received [chemotherapy](https://www.today.com/health/selma-blair-ms-remission-life-update-rcna74226), remarking: “It’s so amazing that there are such old treatments that can be used in ways that are so different than what I thought.”

Kolb, a pediatric hematologist oncologist and researcher, also acknowledged the progress that has been made in medical research—not just in discovering new drugs but also in finding ways to repurpose existing treatments in novel ways. “We have been able to learn a lot about the genetic and molecular underpinnings of the diseases and target them with precision therapy,” he said. These precision treatments that were initially developed for specific conditions like cancer can also be used to understand the immune system as a whole. “We see immunotherapies now being used for lupus or other autoimmune diseases,” he added. Innovations in finding curative therapies as well as implementing better screening and prevention is [improving survival rates](https://www.nfcr.org/blog/7-facts-need-know-blood-cancers/) among blood cancer patients, for example.

However, there’s a caveat. The complexity of care, especially for long-term, chronic illnesses, is [driving up costs](https://time.com/7275808/trumps-tariff-american-health-care-effects/) for patients and their families. Not to mention curative therapies can come with a [hefty price tag](https://time.com/4989537/blood-cancer-gene-therapy/). It’s a situation that Blair is all too familiar with. “The cost for treatment, even for a beautiful, rich, and famous woman like me,” she said, half teasingly, “was really brutal. … The money thing can wear you down, and you are already exhausted.”

One solution to bringing down costs, Diamond noted, is to revamp the approach to primary care. A properly functioning primary care network can help patients track their health holistically, as well as manage their chronic illnesses outside of specialist offices, deburdening specialist workloads. Growing healthtech investments have unlocked a bounty of [new technologies](https://time.com/7203635/our-healthcare-system-is-broken-can-technology-help-heal-it/) and diagnostic tools that could assist providers in catching malignant symptoms early for abnormal conditions. Blair noted that if she got an MRI scan earlier in her care journey, it probably would’ve saved her a lot of doctor’s appointments.

“What primary care does, when it’s going well, is reduce total costs,” Diamond said. “Maybe 20, 25 percent of what we currently spend on health care could have actually been avoided altogether if patients had gotten timely, appropriate care in a primary care setting.”
