How old are you, _really_? People are flocking to so-called [longevity](https://time.com/collections/future-of-living/7341846/aging-research-nir-barzilai-longevity/) clinics and buying tests online to find out their “[biological age](https://time.com/collections/future-of-living/7341841/longevity-age-cells-douglas-vaughan/)”—a metric that some scientists say could reveal how healthy a person really is and serve as a first step to understanding how to help them live healthier for longer. 

Unlike chronological age, which is fixed and based on when you were born, biological age can change and is a reflection of how your body's cells, tissues, and organs are functioning and aging, longevity scientists say. Biological age is one way to gauge a person’s “[intrinsic capacity](https://pmc.ncbi.nlm.nih.gov/articles/PMC8947834/),” or overall mental and physical health, says Tony Wyss-Coray, a Stanford neuroscientist who studies aging. “What is the intrinsic capacity of the cells or tissues in your body? How well are they functioning compared to how old you are?” he says.


The U.S. government recently launched an initiative to identify more accurate and reliable ways to measure biological age. Companies and researchers globally are racing to do the same. The number and diversity of these tools, often referred to as “biological-age clocks,” is dizzying. Some are blood or saliva tests and are based on chemical modifications to [DNA](https://time.com/5783784/dna-testing-genetics/); others analyze protein patterns or data from wearable devices and routine electronic health records. Some purport to measure biological age based on images of a person’s retina or face, or even the sound of their voice and strength of their grip.

“[Biological age](https://time.com/7301118/brain-biological-clock-aging/) is the shiny object on the hill that everyone wants to measure and determine,” says Dr. Eric Verdin, president of the Buck Institute of Aging Research. “There’s not a day that goes by that I don’t see a new clock being developed.” 

Scientists are already using these clocks to study the biology of aging, and health enthusiasts are experimenting with them to measure their own vitality. Verdin says he uses clocks, together with wearable devices and regular blood tests, to get a general sense of how healthy he is. Based on several clocks, his biological age is years younger than his chronological age, he says, which encourages him to continue his [healthy habits](https://time.com/collections/future-of-living/7341844/4-science-habits-longer-life/) of eating a Mediterranean-style diet, prioritizing sleep, and working out regularly.

Tech entrepreneur Bryan Johnson has said he is only aging about six months for every year that passes based on one biological-age test. He [credits his elaborate longevity regimen](https://time.com/collections/future-of-living/7341335/bryan-johnson-longevity/), which includes consuming dozens of supplements and following a strict wellness routine, for this result. 

The problem is, nobody quite knows how these clocks actually work or if they work at all. 

It is unclear whether existing clocks are truly measuring things that are driving biological age or merely things that are correlated with age, says Martin Borch Jensen, founder of Norn Group, a think tank for longevity research, and chief scientific officer of Gordian Biotechnology, a drug discovery and development company. At this point, clocks that people use to appraise their own health are merely a form of “consumer longevitainment,” he says. “What is needed are clocks that are interpretable, which means that there is a clear causal link between the test and biological age.” 


Take the lipid panel, a blood test that measures cholesterol and is used to assess cardiovascular risk. That’s a highly interpretable measurement, Jensen says. Doctors understand the role that cholesterol plays in the body and its relationship to cardiovascular health, making lipid panels accurate and meaningful predictors of heart health. 

No single biological-age clock on the market is currently accurate or interpretable enough to provide truly useful or actionable information at an individual level, Jensen says. But improving clocks so that consumers—and also drug companies—can use them meaningfully is foundational for the future of the longevity industry, many scientists of aging say. 

## Roadblocks to clocks

In recent decades, scientists have discovered that [aging](https://time.com/collections/the-age-of-longevity/7338547/new-years-resolutions-age-better/), previously considered inevitable and unchangeable, might actually be a process that can be modified. Some researchers now consider aging a disease that could be treated, slowed down, or even reversed. Studies in animals have suggested that some genetic mutations, along with interventions including some drugs and lifestyle changes, can slow aging and enhance health span, the number of years spent in good health. 

But without a way to reliably and efficiently measure aging in a human being, clinical trials to test these same interventions in people have been difficult to do. 

In the U.S., a patent on a new drug typically lasts 20 years, but that includes the time needed to conduct clinical trials. That timeline is a problem for any trial that seeks to test a drug that specifically targets aging, Jensen says. Unlike, say, mice, people tend to age very slowly, and “no investor is going to give money to a company to run a trial that is going to take decades,” he says. The alternative is to run a study involving tens of thousands of people, which could amass enough data in the shorter-term to possibly generate a significant result, but such a trial would be prohibitively expensive for many companies, Jensen says. 

Some biotechs and pharmaceutical companies are already exploring the potential of longevity treatments, but they are testing these drugs in age-related conditions such as liver disease or obesity rather than testing them against aging itself. “Companies are doing all those things hoping that the drug may still work for longevity, but no one's actually going all the way and running the test for longevity,” Jensen says. 


A gold-standard biological-age clock or clocks that scientists and regulators both accept as accurate and meaningful would be groundbreaking—and could finally make it feasible for companies to test whether treatments slow aging, says Verdin of the Buck Institute. Until then, he says, “we are all flying blind.”

Validated clocks could also transform health care, Verdin says. “Imagine you are 35, and you go to your primary-care doctor and get your blood drawn, and based on that, your doctor can tell you that your biological age is actually 45,” he says. “The biomarkers might point to certain deficiencies or risk factors, and your doctor can make specific recommendations based on that.”

## A glimpse into the future

Verdin says that while some clocks are useful research tools, none are currently ready for primetime, whether for clinical trials or individual use. Still, some people enjoy tinkering with them and may find themselves motivated to make better lifestyle choices, he says. But Verdin cautions people to take any biological-age result with a grain of salt. 

For now, anyway. 

“My prediction is that we will have tools in the next five years” that will be able to meaningfully measure biological age, Verdin says. 

Several efforts are underway to help make this a reality. The Buck Institute, in collaboration with the research nonprofit Phenome Health, is recruiting people for a [clinical trial](https://www.buckinstitute.org/lab/time-study/) that seeks to identify biomarkers that can reliably predict biological age and diseases of aging. To find these biomarkers, researchers will be using [AI](https://time.com/7346545/ai-claude-cowork-code-chatbots/) to analyze an enormous amount of biological data, Verdin says. “At the Buck, we can measure more than 100,000 different variables: proteins, metabolites, epigenetic marks, lipids, all the data from your wearable devices,” he says. “We are trying to understand which of these markers could be relevant and stable. Once that’s done, we will start identifying which ones are most predictive for disease and for lifespan and so on.”

Some of this work is being funded by the Advanced Research Projects Agency for Health (ARPA-H), a U.S. government agency that launched a new longevity initiative in December aimed at finding gold-standard biological-age clocks that could be used in clinical trials that test anti-aging interventions. ARPA-H says it plans to work with several research groups across the country to identify these clocks through clinical trials and AI-driven analyses of existing datasets.


The goal is to help the [longevity industry](https://time.com/collections/the-age-of-longevity/7338545/the-new-old-age/) realize its promise of [producing therapies that could delay the aging process](https://time.com/collection%5Fhub%5Fitem/longevity-drug-bioage-labs/) by finding quicker and more cost-effective ways to test these interventions in people, says Andrew Brack, a molecular biologist who is leading the initiative, known as PROSPR. “We need a biomarker that can serve as a surrogate for natural aging that could tell you that answer in two or three years. That way, we could get more therapeutics, and they would be less expensive to develop and would get to people quicker,” he says. 

As part of PROSPR, researchers will test promising clocks to see how well they can predict a person’s health in the future. They will also use the clocks to analyze the anti-aging—what scientists call “geroprotective”—potential of several commonly used drugs that preliminary research suggests could have health span-extending properties, Brack says. ARPA-H has yet to identify the drugs that it will test, but GLP-1 drugs, the immunosuppressant rapamycin, and SGLT2 inhibitors, drugs that are used to treat Type 2 diabetes, are potential candidates, he says. 

“Once we've done that trial, it's possible that not only will we have surrogate biomarkers for health span, but we might also identify the first-generation of geroprotective drugs for people,” says Brack, who previously cofounded a longevity biotech and researched aging as a professor at the University of California, San Francisco. “It’s an exciting time for the field.”

Many longevity researchers and companies are devoting funds and attention to building better clocks and figuring out ways to validate them. Norn Group, Borch Jensen’s longevity think tank, is working on developing a testing program that it says could evaluate existing clocks and how accurate and interpretable they are. The Biomarkers of Aging Consortium, a group focused on the creation of proven and standardized biological-age tests, launched a contest in 2024 that pits different clocks against one another. The competition is ongoing. 

“Everybody claims their clock is the best, but there’s no fair comparison of them,” says Vadim Gladyshev, a member of the consortium’s scientific steering committee. “We need standard datasets, analysis, and harmonization.”

Gladyshev, a professor of medicine at Harvard who studies aging and whose lab has developed its own clocks, says he is excited about next-generation clocks that could analyze multiple biological processes and patterns instead of just one. Such clocks would likely be more informative and useful, he says.

Clocks that measure aging of specific organs and not just aging of the entire body could also be transformational, Gladyshev says. Research done by his lab and others has suggested that people’s organs may age at different rates. “Maybe the kidney is aging faster than the brain, for example, and in those cases, we might need interventions that are more personalized and that target specific organs,” says Gladyshev.

Wyss-Coray, the Stanford researcher, is a pioneer of [organ-specific biological age clocks](https://time.com/7301118/brain-biological-clock-aging/). His lab has created a blood test that can estimate how well specific organs are functioning by measuring proteins that are unique to them. Vero Bioscience, a company Wyss-Coray cofounded to help commercialize this technology, is now working with private clinics to test the clock at a small scale in real patients.

“We are working closely with clinicians to assess the ages of a person’s organs,” Wyss-Coray says. “Clinicians make recommendations based on the test, and then we follow the patients and collect data.” 

Larger studies need to be done to truly validate organ-specific clocks and other tests like it. But Wyss-Coray says he thinks such clocks could be used in clinics in a few short years. “The major hope is that we will be able to do preventative medicine and give people real feedback” about whether treatments and other interventions are working to improve their health, he says. “It could completely change medicine.”
