An estimated [400,000 people](http://www.propublica.org/article/how-many-die-from-medical-mistakes-in-us-hospitals) die due to avoidable mistakes every year in American hospitals. These medical errors can’t all be pinned on careless or inexperienced doctors, either. The most experienced medical professionals are less than infallible, beset by distractions, exhaustion, and anxiety. Mainly, though, medical error stems from an innate human flaw: our pathological resistance to noticing, and correcting, our own habitual mistakes.

Every one of us experiences this: When we’re wrong about something—a character judgment, a trivia fact, or perhaps a medical assessment—we’re plunged into cognitive dissonance. Seeing evidence contradict our expectations actually causes us into psychic pain. So we reframe the facts. Our subconscious is in the business of editing reality.


But what intensifies this trait for doctors is the tendency towards evasion deeply embedded in the culture of their profession. In health care, competence is equated with clinical perfection. Amputating the wrong limb or administering the wrong drug isn’t like taking the wrong turn to the supermarket; it is the most serious threat to professional self-worth imaginable. This pressures doctors to keep from admitting failure to their colleagues, patients, and possibly even themselves. They deploy a series of euphemisms—“technical error,” “complication,” “unanticipated outcome”—each of which contains an element of truth, but none of which provides the whole truth.

It doesn’t have to be this way. Aviation offers a different approach to confronting failure. In the wake of a crash or near miss, the aviation industry doesn’t spin the incident to portray itself in a more favorable light, but uses it as an opportunity to learn. [Black boxes](http://www.ntsb.gov/news/Pages/cvr%5Ffdr.aspx) are opened, data is analyzed, and procedures altered so that the same mistakes don’t happen again. Over time, this has had an astonishing impact on safety. In 1912, more than half of U.S. army pilots died in crashes in peacetime. By last year, the accident rate for major airlines had fallen to one crash for every [8.3 million takeoffs](http://www.iata.org/pressroom/pr/Pages/2015-03-09-01.aspx).

Pioneering institutions grasp a precious truth: failure is unavoidable. The key is to harness the lessons of failure as part of a dynamic process of change. Google, for example, runs controlled trials at scale, clocking up thousands of tiny failures, and learning from each. The inventor James Dyson worked his way through [5,126 failed prototypes](http://www.entrepreneur.com/article/224855) before hitting upon the vacuum cleaner design that changed the world.

Pixar goes through an iterative process involving about [12,000 storyboard drawings](http://ink.library.smu.edu.sg/cgi/viewcontent.cgi?article=3852&context=lkcsb%5Fresearch) to make one 90-minute feature. In his autobiography, Pixar’s long-serving president Ed Catmull says: “We are true believers in the power of bracing, candid feedback and the iterative process—reworking, reworking and reworking again, until a flawed story finds its throughline or a hollow character finds its soul.”

Heather Hanbury, a British headteacher, has created an annual event for students to take the stigma out of failure. It is called “failure week” and involves workshops where failure is celebrated. Parents and role models talk about the times they failed, and what they learned from the experience. “You’re not born with fear of failure,” she said. “Young children have no fear of failure at all. They have great fun trying new things and learning fast. Our focus here is on failing well.”

This is the revolution we need today: in our schools, hospitals and lives. It is not easy to engage with error, but it pays dividends every time. Only when we learn to see our mistakes in a clear-eyed way can we break free of a cycle of underachievement and open the door to innovation.
