Innovative and advanced medical practices, technology, and changing lifestyle behaviors, such as increased physical activity and healthier diets, all work together to make living longer a likely life outcome for many of us. These trends, which are on-going, are driving a rethink about the nature and delivery of LTC to a growing elderly population, and raising questions about what elderly care will look like in the future. At the heart of that rethink is how advances in digital technology are changing the face of elderly care as we currently know it. Nevertheless, challenges in LTC remain. These include beliefs and attitudes toward aging, financing healthcare, promoting healthy behavior change, aging in place, and the chronic shortage of trained care workers. One belief that affects our approach to LTC is that being elderly inevitably leads to a diminished quality of life. This belief, especially in the US, has been foundational in our approach to the elderly and LTC, which has to a large extent focused on disease and degeneration management to the exclusion of personal growth and adding value to the elderly years. #### Redesigning LTC In this respect, the US can learn a lot from other approaches, such as those in Japan. Progress in this area includes a mix of initiatives to improve the working conditions and effectiveness of formal and informal caregivers, such as innovative recruitment techniques, raising pay rates, and governments and employers working together to provide more flexible working conditions and compensation for workers who are also in a caring role. The EU, in particular, leads with policies that improve coordination between the various disciplines that LTC draws on as well as between formal and informal caregivers.
