It doesn’t look like much of a marvel. If anything, it appears to be just a bunch of ordinary personal computers, a couple of feet high, wired together and left running in a corner. But stored on disks in these machines is an extraordinary medical cornucopia: the details of thousands of heart attacks painstakingly etched into silicon over nearly 30 years. Each spasm, each chemical released into the bloodstream by a dying heart muscle, each patient’s treatment, is registered in this giant multivariate database by doctors, nurses and researchers at Duke University. The heart of the Medical Center’s Databank for Cardiovascular Disease, these computers tell doctors, with greater certainty and accuracy every day, the best ways to treat not just heart attacks but a variety of other heart problems as well.

Like an economics student analyzing inflation, interest and unemployment rates and coming up with a forecast of which way the economy will go, the Duke databank, now a part of the Duke Clinical Research Institute, correlates important information about the body–enzyme levels, age, family history–into a prediction of how a given patient will respond to a certain type of treatment. Doctors on five continents dial into the system, type in the critical components of their patient’s problem and get a recommendation for treatment based on the thousands of patients who have passed that way before. And of course doctors at Duke routinely use the computer to provide the best treatment possible for their own patients, like Therwell Wrenn (see photo panel next page), an electronic-components designer suffering from coronary-artery disease. To some extent, the bytes and bits stored on those ordinary-looking machines have taken the guesswork out of cardiology.

Though that may seem extraordinary, such major advances in the collection and analysis of patient information are becoming commonplace in modern medicine; technology has made the impossible probable and the unlikely a certainty. Twenty years ago cardiologists mostly guessed at what caused heart attacks. Now young heart doctors, using chemicals, nuclear imaging devices and databases like the one at Duke, can diagnose and treat heart attacks with astonishing efficiency.

In specialty after medical specialty, doctors say, the accelerating pace of invention is saving countless lives in hospitals as well as changing the way people live outside them. “These developments aren’t just evolutionary,” says Rebecca McKenzie, a head nurse who has been at Duke for the past 10 years and remembers the days when each patient traveled with a paper file six inches thick. “They’re revolutionary.”

For better or worse, the driving force behind that revolution is pure economics. Gamma-radiation knives, wondrous devices that focus tiny cobalt beams precisely on microscopic brain malignancies and malformations, cost $3 million each but may ultimately reduce the need for other costly therapies and thus afford a net saving to society. Sophisticated scanning devices–computerized axial tomography (CAT), magnetic resonance imaging (MRI) and nuclear-imaging systems–cost hospitals millions of dollars, and patients (or their insurers) are typically charged thousands for their use. But by pinpointing hard-to-find tumors and other signs of disease, these machines save invaluable time and further diagnostic testing.

At Duke great savings are found in relatively simple numbers. Doctors there surveyed the thousands of cases in the cardiology databank in the 1970s to determine whether the length of time spent in the hospital after a heart attack affected survival rates. In the vast majority of cases, analysts concluded, it was safe to send patients home after only a week in the hospital. That recommendation cut the traditional “watch and wait” period in half–and potentially slashed billions of dollars in in-patient costs.

With the three-pronged motivation to save money, time and lives, physicians, engineers and M.B.A.s–sometimes all rolled into one person–have been starting up hundreds of medical-technology firms to develop new machines. Dr. Steven Charles, a Memphis eye surgeon who also has degrees in mechanical and electrical engineering, has developed a system that greatly enhances the dexterity of surgeons, for example. Since even the steadiest hands oscillate with each heartbeat, there is an absolute limit on human accuracy in microsurgery. In operations performed at less than 10 to 20 microns, a fraction of the thickness of a human hair, even the tiniest hand movements are like earthquakes, tearing flesh and cutting delicate nerves.

Charles’ idea was to use a remote-control device, like the ones kids use to steer model cars, to guide a scalpel into microscopic operating spaces. Much the way shock absorbers cushion an automobile from bumps and potholes, a program that controls the variation between the remote control and the platform-mounted scalpel helps cushion the natural vibrations in the surgeon’s hands. With the help of scientists from the space-age Jet Propulsion Laboratory in Pasadena, California, Charles is developing a special microdexterity system for surgeons. Once the tool is perfected, it will be put to work operating deep inside the brain, on nerve endings at the base of the spine and in delicate areas inside the ear. “This isn’t robotic surgery,” Charles points out. “It does for your hand what a microscope does for your eye.”

Computers are helping doctors not only to move better but to think smarter. Among the latest trends in electronic medicine is the development of computer-based “expert systems” that help doctors diagnose diseases. Doctors punch in the details of a patient’s condition–or in some instances wire the patient to the computer–and then wait for answers to the trickiest clinical problems. How smart are these machines? The first expert system, developed at Stanford University in 1970, diagnosed a range of different infections and prescribed appropriate antibiotic treatment. In controlled tests the digital doctor bested the performance of nonspecialist physicians and, 9 times out of 10, equaled that of the specialists.

In the past 25 years these thinking machines have only got smarter. POEMS (Post Operative Expert Medical System), a program developed at St. James University Hospital in Leeds, England, helps less experienced doctors treat patients who get seriously ill during recovery from surgery. The computer watches a collection of information about the patient and then encourages doctors and nurses to take a specific action.

Once that kind of intelligence is wired into a computer, the next logical step is connecting the system to the outside world. In fact, “tele-medicine,” as doctors refer to a range of such long-distance ministrations, is the latest buzz in medical technology. The idea is simple enough: doctors and computers in advanced research centers should be able to “dial in” to rural areas to diagnose and treat patients, opening up a whole new medical frontier.

Dr. Lester Weiss, a geneticist based in Detroit, spends much of his time these days “visiting” his patients via telephone. He calls not to chat but to analyze the details of their care through telemedicine links between Henry Ford Hospital and clinics across rural Michigan. Weiss runs Ford Hospital’s Genetics and Birth Defects Center, a regional research hub. Telemedical technology brings his world-class experience to locales hundreds of miles afield–and to more than one place at once.

Sitting in his office, Weiss scans a computer-enhanced image of a newborn baby lying in intensive care in Saginaw, Michigan, 100 miles north of Detroit. The child was born wrapped inside a rare membrane that causes its skin to peel away in gobs, leaving the baby dangerously exposed to infection. Viewing a high-resolution picture of the youngster sent to the hospital and displayed on a PC, Weiss is able to diagnose the condition, known as lamellar ichthyosis, and suggest a treatment. “It was pretty horrendous,” he says of the infant’s condition. “But we were able to tell them what it was and how to treat it without having to transfer the baby.”

Similar databanks at other medical centers have an even broader reach. At the University of Iowa, researchers have assembled an online hospital that links more than 50,000 doctors and patients a week. The system is so sophisticated that a doctor in India can log on and actually see and hear the symptoms of a child with whooping cough, entered online by his Iowa colleagues, enabling him to compare them with those exhibited by his own young Indian patient. The multimedia Web pages are literally saving lives.

The idea of transferring knowledge instead of patients may be the most important medical trend of the next decade. As the world becomes increasingly interconnected, such information and expertise warehouses as the Duke Databank and Henry Ford Hospital will become resources for doctors all over the world. Knowledge that once took decades or years to spread from research centers to practicing physicians now moves in hours.

“We can’t use textbooks anymore in our field because they’re dated by the time they come out,” Weiss explains. “Some monthly journals are too old because of how rapidly things are changing in our field.” In a profession where seconds can mean the difference between life and death, the instant access to information provided by the new machines may one day save as many lives as any miracle drug.
