How much protection do you currently have against COVID-19? The answer depends on so much: whether you’ve [already had COVID-19](https://time.com/6177192/covid-19-reinfection-how-often/) (and if so, how long ago); whether you’ve [been vaccinated](https://time.com/6140281/covid-19-immunity-boosted-infected-safety/) (and if so, how many times, and how recently); whether you have any medical conditions that could weaken your immune system; and so on. Even [antibody testing](https://time.com/5880255/covid-19-tests-types/) only approximates immunity to COVID-19, so there’s no simple way to know.

But an international group of researchers recently developed a different tool to help assess COVID-19 immunity: a blood test that can measure T cells, white blood cells that work alongside virus-fighting antibodies to mount an immune response. Their work is described in [a new study](https://www.nature.com/articles/s41587-022-01347-6) published in _Nature Biotechnology._

T-cell testing for the SARS-CoV-2 virus isn’t totally new—the U.S. Food and Drug Administration [granted emergency-use authorization](https://www.fda.gov/news-events/press-announcements/coronavirus-covid-19-update-fda-authorizes-adaptive-biotechnologies-t-detect-covid-test) to another T-cell test, called T-Detect, last year—but it tends to be labor- and time-intensive, says study co-author Ernesto Guccione, a professor of oncological sciences at New York City’s Tisch Cancer Institute at Mount Sinai. His group, which includes other researchers from Mount Sinai and institutions including Singapore’s Duke-NUS Medical School, aimed to simplify it by using technology that is widely accessible and can turn around results in less than 24 hours.

Their process starts with mixing a person’s blood sample with material from the SARS-CoV-2 virus. If there are T cells specific to SARS-CoV-2 in the blood, they’ll react to the viral material and produce a substance that can be detected via polymerase chain reaction (PCR) technology, like that used to run COVID-19 diagnostic tests. Measured levels of that compound serve as a proxy for cellular immunity.


The test has been licensed to the U.K.-based biotechnology company Hyris and is already in use in Europe. The FDA is still reviewing the technology, so it’s not yet available in the U.S. (Hyris employees served as co-authors on the _Nature Biotechnology_ paper, and its technology was used in the study; Guccione consults for therapeutic companies and receives compensation through a licensing agreement between Hyris and Mount Sinai.)

Why test for T cells when antibody tests are already widely available? There are a few reasons, Guccione says.

Testing for antibodies only tells part of the story, since T cells are also a critical piece of the body’s immune response. And while antibody levels drop off significantly within a few months of vaccination or infection, cellular immunity can last up to a year, Guccione says. “Monitoring both will give us a much clearer picture \[of immunity\] and will hopefully inform our re-vaccination strategies,” Guccione says. The wide use of this test could help define how long protection lasts and [how often booster doses are needed](https://time.com/6164976/covid-19-booster-shot-strategy/).


Plus, some immunocompromised people do not produce antibodies—even after multiple vaccine doses—but they usually do have some T cell response. T-cell testing could help those individuals learn whether they have any defenses against COVID-19.

There are limits to what the test can reveal. Experts are still trying to find what are known as “correlates of protection” for COVID-19: measurable indicators that suggest an individual is protected enough that they are unlikely to get sick. For now, testing for antibodies or T cells [can’t give you a yes-or-no answer](https://time.com/6171943/how-long-does-covid-immunity-last/) about whether it’s safe to go to a concert or party without fear of catching the virus, for example. It just gives you one more data point to factor into your risk calculation.

Guccione says widespread T-cell testing could help define those elusive correlates of protection by making immunity easier to study. A scalable, affordable testing strategy could facilitate larger studies that bring much-needed answers.


“With large numbers comes clarity,” Guccione says. “That’s the hope: by using this test, we can finally get those numbers that were totally unavailable with the previous technology.”

Spike, surge and uptick are used fairly interchangeably. I think many people in healthcare would view a surge as something that has the potential to overwhelm the healthcare system or threaten the capacity of hospitals in a given area. Whereas an uptick, or an increase, or spike in cases is usually used a little bit more casually just to suggest that case counts are rising. The ways that the U.S. has been predicting when a spike is coming, one of them is just looking to other parts of the world. Europe and particularly the U.K., has been a good predictor for the U.S. throughout the pandemic, they started to see Delta cases and Omicron cases rising a few weeks before we did.

So, looking at what's going on in other countries is very valuable, as is looking at metrics like testing. If more cases are being reported and if a higher percentage of tests are coming back positive, those are both indications that there's more virus spreading in the community. Scientists and researchers all over the world are monitoring COVID, of course. Particularly looking for new variants as the virus continues to evolve. And a lot of those efforts are coordinated through the World Health Organization. But also, independent labs are scientists who are sequencing COVID samples to look for new variants, will often sound the alarm for the rest of the world when they start to notice either the viruses evolving or that a certain variant is spreading more rapidly in certain area.

For example, before the Omicron surge, that news came out of South Africa originally and that kind of let the rest of the world know that they needed to be aware this variant was coming. During a spike in cases, I think we can keep doing the things that health experts have really been recommending for the last couple years. A lot of vaccine and mask mandates have rolled back recently but that doesn't mean that masking and vaccination are not still effective or still necessary in certain situations. So if case levels are rising, or very high in your area, you might want to wear protective masks like a N95.

When you go out into indoor areas in public, you might want to limit the time that you spend in public indoor areas. You should stay up to date on booster shots as they're recommended for different demographics. And you can take advantage of that home tests to test yourself before you gather with others to make sure that you're not unknowingly infected with the virus.
